Report your child's absence online

Please use the online form below to report a child's absence - please note that this form must be used to report sickness absence only. 

• If your child needs to attend a medical appointment, please inform the school in advance and provide the school office with proof of the medical appointment (appointment letter/card).
• For any other reason for absence, please complete a request for leave from. 

You need to complete the form by 8.30am on each day of the child's absence.

If you complete the form you do not need to call the school office to notify an absence, but the school may call you back if we require any further information.
